insurance and payment
The annual membership fee includes your Cleveland HeartLab blood work, all pre-wellness testing, and all of the benefits listed under Our Practice Model. We do bill your insurance and collect co-pays as is required by Medicare and commercial insurers.
Your insurance will not cover the membership fee, although you may be able to use money from a Health Savings Account or Flexible Spending Account to pay for it.
Payment is expected at the time of service. We accept cash, checks and most major credit cards.
We participate with Medicare, some Medicare Advantage plans and most commercial insurances. We do not participate with Medicaid or Workers' Compensation.
For people without insurance, we have a special program that allows you to get steeply discounted labs, imaging, and medication. Your membership gives you access to those benefits, in addition to the ones you will receive as a member.
